Where do in-person appointments take place?
In-person services are available at our home office in Long Beach and in select professional offices in Los Angeles and Orange County.
Concierge services are available for individuals seeking care in their preferred environment or a location closer to home. Appointments may be provided at your home, at the workplace, in residential care settings, or other appropriate locations, offering a personalized and convenient approach to care. Concierge services are available on a case-by-case basis for an additional fee to account for travel time and related costs. Fees vary based on location and will be discussed prior to scheduling.
Telehealth appointments are available to all California residents, providing convenient access to care from the comfort and privacy of your home.
What types of evaluations are NOT offered?
We do not offer evaluations for Autism Spectrum Disorder or intellectual/learning disorders as a primary concern.
What paperwork do I need to complete?
You will receive the following documents electronically to complete before your first appointment:
- New Patient Registration and Informed Consent Forms
- Financial Agreement & Credit Card Authorizations
- HIPAA Notice of Privacy Practices
- Excel Neuropsychology Practice Policies
- Release of Information (as needed)
How should I prepare for my evaluation?
There is generally very little you need to do to prepare for your appointment. The following recommendations can help ensure that testing proceeds smoothly:
- Aim for a full night's sleep before your evaluation.
- Continue taking your prescribed medications unless we have discussed a different plan.
- Bring any glasses, contact lenses, hearing aids, or other assistive devices you regularly use.
- If your appointment is scheduled for an extended period, bring any medications/food/snacks you may need during the day.
- No special studying or preparation is required—simply arrive as you normally would.
Please inform us in advance if you use any medications and/or substances that could affect attention, memory, or thinking skills, as this information may be important when planning your evaluation. In some cases, understanding cognitive functioning without medication is clinically helpful. If that applies to your evaluation, we will discuss the options together and develop a plan that is safe, appropriate, and tailored to your needs.
Do you accept insurance?
As an out-of-network practice, services are not billed directly to insurance. This model allows for greater flexibility, privacy, and individualized care, without the constraints of insurance-driven limitations on diagnosis, documentation, denial of your claim, or treatment frequency. Upon request, a superbill (a receipt of services) can be provided for you to submit to your insurance company for possible out-of-network reimbursement.
What are your rates and when am I billed for services?
- Intervention: $300 per session (50 minutes); payment is due at the time of each session.
- Assessment: $2,500 to $5,000+, depending on complexity and scope. A $1,500 deposit is required at the time of scheduling your evaluation. The final payment is due before the feedback session.
- Concierge Services: Rates available upon request.
- Accepted forms of payment: major credit/debit cards, FSA/HSA card, ACH, cash, and checks.
What is your cancellation policy?
Because appointment times are reserved exclusively for you, please provide at least 72 hours' notice if you need to cancel or reschedule. Cancellations made more than 72 hours before the appointment will receive a full refund. Otherwise, applicable fees will apply.
What is a Good Faith Estimate and HIPAA Notice of Privacy Practices?
A Good Faith Estimate is a document that outlines the expected cost of your care so you have a clear understanding of anticipated fees before services begin. This helps ensure transparency around pricing and allows you to make informed decisions about treatment.
The HIPAA Notice of Privacy Practices explains how your health information may be used and shared, as well as your rights regarding your personal medical and psychological information. It is designed to protect your privacy and ensure your information is handled in accordance with federal law.
